BACKGROUND CHECK - Release Authorization

The information below is required by law enforcement agencies and other entities for positive identification purposes when checking public records.  It is confidential and will not be used for any other purposes. The scope of this notice and below authorization is not limited to the present and, if you volunteer, will continue throughout the course of your volunteering and allow LifeChurch to conduct future screenings every three years for retention, promotion or reassignment, as permitted by law and unless revoked by you in writing.
By signing below, I hereby release all persons, agencies, and entities providing information or reports about me from any and all liability arising out of the requests for or release of any of the above mentioned information or reports, and I authorize the obtaining of a national criminal and sex offender background check now, and at regular three year intervals, by LifeChurch any time after receipt of this authorization and throughout the course of assignments, as applicable.
